HeartNetCare-HF

HeartNetCare-HF is a disease management programme for patients with systolic heart failure (HF), which was validated in a large randomised trial within the German health system. It is based on previously successful modules and considers patients’ preferences.

The programme features several elements, including telephone-based monitoring and nurse-coordinated care. It provides patients, carers and families with teaching materials to educate them on self-monitoring protocols and involve them in care.
