HF integrated programme, Maastricht University Medical Centre

The Maastricht University Medical Centre developed a programme that integrates primary and secondary heart failure (HF) care. It includes visits from specialist clinicians to primary care practices to educate primary care clinicians about optimal care. The specialists collaborate with primary care practices making use of the HF care programme.

The Heart Failure Policy Network is an independent, multidisciplinary platform made possible with financial support from AstraZeneca and Roche Diagnostics. The content produced by the Network is not biased toward any specific treatment or therapy. All outputs are guided and endorsed by the Network’s members. All members provide their time for free.
