HF care pathway with telemonitoring, University Medical Centre Utrecht

The University Medical Centre Utrecht has developed a care pathway where a heart failure (HF) specialist sees people living with HF in an outpatient clinic.

The pathway includes a telemonitoring component to support monitoring at home, and it may be invasive (via an implanted device) or non-invasive. The specialist nurse monitors signs and symptoms of HF, provides self-care education and support, and titrates medication if needed. The service is supervised by HF specialist cardiologists, and involves other professionals such as GPs, social workers and dietitians. The pathway also offers access to cardiac rehabilitation.
