Palliative services, St Christopher’s Hospice

In 2011, St Christopher’s Hospice in London refined its palliative services with a particular focus on heart failure (HF) management and supportive palliative care.

A central aspect of the intervention is the rehabilitative palliative care service, which supports individuals to achieve personal goals. Patients and carers value the personal and flexible service, which helps them according to their needs and not within a set review schedule.
