Redesigning the HF care pathway, NHS Foundation Trusts

Two London hospital trusts (Guy’s and St Thomas’ NHS Foundation Trust and King’s College Hospital NHS Foundation Trust) are developing a project to redesign the heart failure (HF) pathway. The aim is to ensure close liaison between specialists and GPs managing people with HF, and to develop education sessions and ‘virtual clinics’.

The project, which will cost £1.5 million, aims to reduce costs to the National Health Service by approximately £500,000 a year, save up to 80 lives a year, reduce annual bed days by 3,000 and improve quality of life for people with HF.
