Heart Failure Liaison Nurse service

NHS Glasgow’s Heart Failure Liaison Nurse service assigns a specialist heart failure (HF) nurse to follow up on patients and act as a link between primary and specialist care.

People with HF who are admitted to hospital are assigned to a HF nurse. The nurse follows up with people following discharge from hospital, adjusts medication and acts as a coordinator between the person with HF, their GP and cardiologists.
