The Heart Failure Policy Toolkit is a policy resource aiming to expose existing gaps in heart failure care and offer integrated, sustainable solutions.

What is the Heart Failure Policy Toolkit?

The Heart Failure Policy Toolkit was published in 2015 as an interactive online resource. It focused on priority actions organised around four key themes:

  • Make heart failure a national priority
  • Diagnosis and prevention in those at risk
  • Multidisciplinary, person-centred care
  • Investment in tools and resources in primary care.

Members and supporters of the HFPN gathered at the European Parliament on World Heart Day (29 September 2015) to launch the Heart Failure Policy Toolkit along with the HFPN’s call to action.

Who is the toolkit for?

The Heart Failure Policy Toolkit was created to support advocacy around the HFPN’s initial call to action. It provided information and resources for policymakers, patient organisations, healthcare professionals and anyone advocating for policy change in heart failure across Europe.

Based on the four key themes of the policy toolkit, the HFPN called for action in 10 areas:

  • National HF strategies with measurable goals
  • Public awareness campaigns
  • Data-driven resource planning
  • Professional training
  • Availability and reimbursement of diagnostics
  • A standardised HF pathway that reflects clinical guidelines
  • A dedicated contact for follow up care
  • Person-centred care plans focused on improving quality of life
  • Workforce planning in primary care
  • Investment in innovative technologies that promote self-care

The Heart Failure Policy Network is an independent, multidisciplinary platform made possible with financial support from AstraZeneca and Roche Diagnostics. The content produced by the Network is not biased toward any specific treatment or therapy. All outputs are guided and endorsed by the Network’s members. All members provide their time for free. The Network is hosted by The Health Policy Partnership .
