A blueprint for integrated, multidisciplinary heart failure care

In 2006, the Italian Association of Hospital-Based Cardiologists (AMNCO) developed a heart failure integrated care pathway that can be adapted to different contexts depending on available resources.

It aims to offer regions or localities a guide to develop a coordinated response to patient needs over the course of their condition.
